THRIVE – WELLBEING PROGRAM

Our Mission: We want to help our associates live longer, healthier, happier, more productive lives both at work and away from the job creating a culture of wellbeing throughout all departments with encouragement, education, and support for each associate’s wellbeing.

Earn a wellness credit of up to $100 per month toward your medical premium!  Not enrolling in medical benefits? You can still earn points toward gift cards!

Equity is dedicated to the wellbeing of our associates and recognizes the importance of choosing a healthy lifestyle. Through our wellbeing program THRIVE, we incentivize associates and their families to make positive health choices. A component of the program encourages associates and their covered spouses to live healthier lives by completing age-appropriate physical exams and screenings. Associates enrolled in the Medical Plan who complete the following requirements through the Vitality portal  www.powerofvitality.com, within the current plan year, will be eligible to receive a premium adjustment for the following plan year.

Associates without a Spouse on the Medical Plan:

Associates with a Spouse on the Medical Plan:

  • Non-tobacco, $25/month
  • Gold Status & 2 Healthy Ranges, $25/month
  • Platinum Status & 3 Healthy Ranges, $50/month
  • Non-tobacco, $25/month
  • Gold Status & 2 Healthy Ranges, $25/month
  • Platinum Status & 3 Healthy Ranges, $25/month
  • Spouse completes Preventive Care Form, $25/month

The amount of the premium adjustment will depend upon the number of the above items completed by an associate and/or spouse.
