Associate Health Care Premiums

Associates are asked to share the cost of their insurance benefits with the Company. Your contribution rates are outlined in the tables below. The contribution rates are based upon semi-monthly payroll deductions (24 pays).

The 2025 insurance premium deductions will start on the January 3, 2025 pay date.

* Associates who participate in the Vitality wellbeing program may be eligible for a wellness credit of up to $100 per month on medical premiums.  (See page #14 for more information)

SECTION 125 PLAN OVERVIEW

Equity Administrative Services, Inc. will continue to offer an Internal Revenue Code (IRC) Section 125 Premium Conversion Plan that allows you to pay for your healthcare contributions on a pre-tax basis.

By paying your premiums with pre-tax dollars, you can reduce the amount of income and Social Security taxes that you otherwise would be required to pay. The elections you make during the Open Enrollment period are effective for the entire 12-month plan year, January 1st through December 31st. You can only make a change to your elections during the year if you experience a qualifying event (refer to Eligibility Overview for the definition of qualifying event on page #3). If you believe you have experienced a qualifying event and you wish to change your elections, you are required notify HR within 30 days of the qualifying event.
